We seek a dedicated Care Manager to collaborate with our team in developing customized care plans for our patients.
Care Managers play a pivotal role in coordinating patient care, liaising closely with families to oversee recovery progress, and facilitating access to appropriate healthcare providers as required.
Your primary responsibility will involve providing direct patient care to enhance their well-being during their stay in our healthcare facilities.
Oversee and execute key operational duties to ensure organizational efficiency, including managing daily workflows, coordinating team efforts, and maintaining high performance standards. Develop and implement strategies to optimize processes, enhance productivity, and support long-term goals. Collaborate with cross-functional teams to align objectives, resolve challenges, and drive continuous improvement. Monitor progress through performance metrics, adjusting approaches as needed to meet targets and exceed expectations. Ensure compliance with company policies, industry regulations, and best practices while fostering a culture of accountability and innovation.
Engage collaboratively with healthcare professionals to streamline and enhance patient treatment plans.
Ensure that all patient treatment needs are fulfilled in accordance with prescribed protocols and clinical standards.
When patients’ requests for services fail to align with established medical necessity criteria, healthcare providers should propose alternative treatment options to ensure appropriate care while adhering to guidelines.
Instruct and educate patients regarding medical procedures, healthcare provider directives, and referral processes to ensure a clear understanding and adherence to prescribed care plans.
Establish connections between patients and essential social services programs, including transportation assistance and translation services, to ensure they receive the necessary support.
Perform consistent patient follow-ups to assess advancements, foster ongoing care, and guarantee enhanced health outcomes.
Responsible for documenting and updating case management activities in accordance with established protocols.
We are seeking a highly skilled professional who possesses a strong educational background in a relevant field, complemented by substantial experience in the industry. The ideal candidate will demonstrate expertise in key responsibilities such as project management, team leadership, and strategic planning, alongside proficiency in industry-specific tools and methodologies. Excellent communication skills, both written and verbal, are essential, as is the ability to collaborate effectively with cross-functional teams. Additionally, the candidate should exhibit a proven track record of problem-solving, adaptability, and a commitment to continuous professional development.
Demonstrated success in a Care Manager capacity or an equivalent professional role is required.
Skilled in utilizing Healthcare Management Systems alongside Microsoft Office applications, specifically Word and Excel.
Exceptional proficiency in both written and verbal communication is required.
Proven ability to manage multiple tasks efficiently, prioritize workload effectively, and maintain meticulous attention to detail in fast-paced environments.
Capable of undertaking local travel as needed for the role, with flexibility to accommodate business requirements and occasional travel demands.
Certifications or specialized training in care management are required for this position.
