Job Summary

The Hospitalist oversees the comprehensive management of hospitalized patients from admission to discharge, prioritizing the delivery of superior, well-coordinated inpatient care. This position is dedicated to enhancing patient outcomes while optimizing the utilization of hospital resources through efficient and effective clinical practices.

The Hospitalist collaborates with diverse healthcare teams to oversee the treatment of both acute and chronic medical conditions in an inpatient environment.

This position demands a blend of high-level clinical expertise, operational effectiveness, and seamless collaboration with cross-functional teams to elevate patient safety, care experience, and seamless continuity of services.

Oversee a range of critical duties encompassing strategic planning, operational execution, and performance evaluation to ensure organizational goals are met efficiently. Collaborate with cross-functional teams to streamline processes, identify growth opportunities, and implement best practices that enhance productivity and innovation. Monitor key performance indicators, generate detailed reports, and provide data-driven insights to senior leadership to inform decision-making. Develop and maintain robust policies, procedures, and documentation to uphold compliance with industry regulations and internal standards. Lead and mentor team members, fostering a culture of accountability, continuous improvement, and professional development while aligning individual objectives with broader company objectives.

Deliver thorough and attentive medical treatment to hospitalized patients, ensuring their comprehensive care throughout their stay.

Perform comprehensive patient evaluations during initial consultations, formulate individualized treatment strategies, and oversee continuous therapeutic interventions to ensure optimal health outcomes.

Perform daily patient rounds and maintain precise documentation of clinical observations.

Observe and evaluate the ongoing status of patients, modifying therapeutic interventions whenever appropriate.

Provide care for patients experiencing acute illnesses, worsening of chronic conditions, and recovery from surgical procedures.

Respond immediately to urgent situations involving inpatients and instances of clinical decline.

Promote adherence to clinical protocols and ensure the implementation of evidence-based practices in all patient care activities.

Oversee admission processes, including clerking and documentation.

Conduct daily ward rounds and oversee the timely evaluation of all patients.

Facilitate discharge planning processes to guarantee smooth, secure, and prompt transitions of care for patients.

Craft detailed discharge summaries, issue prescriptions, and develop comprehensive follow-up care plans.

Work collaboratively with specialists, nurses, pharmacists, and allied health professionals to deliver comprehensive patient care.

Collaborate with other departments by initiating and managing referrals and consultations to ensure seamless communication and continuity of care.

Facilitate seamless transitions of care across emergency, inpatient, and outpatient settings to maintain consistent and high-quality patient support.

Take part in collaborative case evaluations and interdisciplinary dialogues to ensure comprehensive understanding and coordination of patient care plans.

Create scientifically validated protocols and guidelines, then instruct medical officers and nurses in their practical implementation within clinical settings.

Comply with established clinical protocols, guidelines, and evidence-based standards of care.

Engage in quality enhancement projects and systematic clinical evaluations to ensure adherence to best practices and standards.

Monitor quality improvement and patient safety indicators including but not limited to medication errors, readmissions, adverse events).

Ensure adherence to infection prevention and control protocols and guidelines at all times.

Document clinical incidents and engage in root cause analysis (RCA) to identify underlying causes and implement corrective actions.

Ensure medical records are meticulously maintained, complete in all necessary details, and updated promptly to reflect the most current patient information.

Effectively convey medical information, including diagnoses, treatment options, and prognoses, to patients and their families in a clear and compassionate manner.

Ensure comprehensive and concise handovers are conducted at the end of each shift.

Facilitate compliance with accreditation standards, such as those set by the Joint Commission International.

Maintain transparent and effective communication with patients and their families about diagnoses, treatment options, and expected outcomes.

Exhibit empathy, mutual respect, and professionalism consistently in every interaction.

Facilitate seamless patient transitions from the point of admission through to discharge, ensuring efficient and uninterrupted care coordination throughout the entire process.

Participate in improving patient satisfaction (CSAT) and experience (NPS) scores.

Oversee and direct the activities of medical officers to ensure adherence to established protocols and standards of care.

Engage in the clinical education and mentoring of junior physicians as part of their professional development and training.

Provide guidance and instruction to clinical personnel to enhance their professional development and ensure proficiency in their roles.

Attend and actively engage in clinical meetings, case reviews, ecosystem grand rounds, and various departmental activities to contribute to collaborative discussions and decision-making processes.

Participate actively in ongoing professional development activities to maintain current knowledge of best practices and evolving medical guidelines.

Ensure compliance with established hospital policies and clinical governance frameworks.

Provide support in enhancing bed allocation and streamlining patient movement throughout the facility.

Engage in scheduled on-call duties and rotating shift assignments.

Requirements

Hold an accredited medical degree, such as an MBBS or equivalent qualification, from a recognized institution.

Completion of the National Youth Service Corps (NYC) is a prerequisite.

Seeking candidates with a minimum of five to seven years of hands-on clinical experience.

Holds an active medical license in good standing.

Proven expertise in clinical assessment and diagnostic evaluation.

Proficient in advanced life support techniques is essential.

Proficient in utilizing computers and associated software applications is required.

Exceptional verbal and written communication skills, coupled with strong interpersonal competencies, are essential for fostering productive collaborations and cultivating positive professional relationships.
