The Linkage Coordinator plays a pivotal role in enhancing HIV testing, referral, linkage, retention, and community support systems to ensure seamless access to HIV prevention, care, and treatment services. This position oversees the implementation, monitoring, and evaluation of referral and linkage processes between communities and healthcare facilities, guaranteeing that newly identified individuals living with HIV (PLH IV) are promptly enrolled in care and consistently retained on treatment. The role involves close collaboration with healthcare providers, community-based organizations, faith-based groups, support networks, and local structures to facilitate client follow-up, adherence support, positive health dignity and prevention (PHDP) interventions, and continuous care services. Additionally, the incumbent contributes to capacity building initiatives, community mobilization efforts, data management, and reporting to drive improved program outcomes and strengthen HIV service delivery across both facility and community settings.

Plays a key role in designing, executing, monitoring, and assessing HIV referral and linkage systems within healthcare settings and community environments.

Facilitates seamless coordination of all referral processes and monitors outcomes between health facilities and communities to ensure continuous, effective collaboration.

Delivers assistance to newly diagnosed people living with HIV (PLH IV) throughout linkage processes, focusing on evaluating and addressing potential barriers that may impede their access to services.

Execute integrated community outreach initiatives to facilitate the seamless connection of testing and treatment programs.

Collaborates closely with HCT teams to ensure prompt follow-up for HIV clients, employing home visits, phone calls, and text messages to track and document process outcomes effectively.

Establish and lead community support groups specifically for people living with HIV, collaborating closely with NEPHRON and other civil society organizations to enhance coordination and impact at state, local government area, health facility, and community levels.

Ensure the successful implementation of community adherence monitoring initiatives.

Implements Positive Health Dignity and Prevention (PHDP) strategies effectively within community settings to ensure comprehensive and sustainable outcomes.

Collaborates extensively with healthcare professionals, community-based organizations (CMOS), and faith-based organizations (FBS) to maintain patient engagement and continuity of care throughout the treatment journey.

Ensure seamless assisted referrals for newly identified People Living with HIV (PLH IV) between community-based and healthcare facility systems while maintaining consistent client retention within the health facilities.

Collaborates closely with testing teams to guarantee seamless patient progression through the care continuum and to enhance client retention.

Implements strategic HIV referral and linkage protocols at AHF health facilities to ensure seamless coordination and adherence to established healthcare standards.

Facilitates the enhancement of competencies among healthcare workers (HCWs) and staff of community-based organization (CBO) partners, specifically in the areas of referral and linkage processes.

Collaborates effectively with key community stakeholders, including the Ward Development Committee (WDC), Village Health Workers (VHW), Health Committees (HC), and Community Development Groups (CDGs).

Ensure the proper management of data and oversee the accurate collection of data at both the facility and community levels using appropriate data tools.

Assist with additional tasks and responsibilities as assigned by management, as required.

Bachelor’s degree in Finance, Accounting, Business Administration, or a related field is required, complemented by a minimum of three years of progressive accounting experience. Proficiency in GAAP standards, financial reporting, and tax compliance is essential. Strong analytical skills, meticulous attention to detail, and the ability to work under strict deadlines are critical. Excellent communication and organizational abilities are also necessary to collaborate effectively with cross-functional teams. Experience with ERP systems, particularly SAP, and advanced Excel proficiency are preferred qualifications.

Seeking candidates with a strong educational background or relevant professional experience. A bachelor’s degree in a related field, such as B.Sc. or BA, or an equivalent qualification is required.

Requires a minimum of two to three years of direct experience delivering HIV-related services in both community-based and healthcare facility settings.

Familiarity with collaborating with CBO partners is advantageous.

Proficient in multiple languages, including but not limited to English, with fluency in verbal and written communication. Strong interpersonal abilities to engage effectively with diverse teams and clients across international markets. Exceptional translation and interpretation skills to bridge communication gaps and ensure clarity in cross-cultural interactions.

Proficient in English communication, both written and verbal.

Proficiency in reviewing, evaluating, and comprehending standard scientific and technical journals, financial statements, and legal documentation is essential.

Must possess strong skills in addressing routine customer inquiries as well as complaints directed from customers or regulatory authorities.
