Job Summary:

The Linkage Coordinator plays a pivotal role in enhancing HIV testing, referral, linkage, retention, and community support systems to guarantee uninterrupted access to HIV prevention, care, and treatment services. This position oversees the execution, monitoring, and assessment of referral and linkage processes between communities and healthcare facilities, ensuring newly diagnosed individuals living with HIV (PLH IV) are seamlessly enrolled in care and consistently maintained on treatment. Collaboration is central to this role, as the Coordinator partners with healthcare providers, community-based organizations, faith-based groups, support networks, and local structures to promote client follow-up, adherence support, Positive Health Dignity and Prevention (PHDP) initiatives, and comprehensive care services. Additionally, the role enhances program effectiveness through capacity strengthening, community engagement, data management, and reporting, thereby bolstering HIV service delivery at both facility and community levels.

Overseeing and executing a range of critical tasks, this role encompasses key duties and responsibilities essential to the position’s success. Duties include managing daily operations, ensuring adherence to company policies, and maintaining high standards of efficiency and accuracy. Additionally, the position requires collaborating with cross-functional teams, addressing operational challenges, and providing strategic input to enhance productivity. Responsibilities involve monitoring performance metrics, identifying areas for improvement, and implementing solutions to drive organizational growth. Plays a key role in designing, executing, monitoring, and assessing HIV referral and linkage procedures within healthcare settings and community environments.

Ensures seamless coordination of all referral processes and meticulously monitors outcomes between health facilities and communities.

Offers assistance to newly diagnosed individuals living with HIV (PLH IV) throughout the linkage process, identifying and addressing potential obstacles to seamless engagement in care.

Execute comprehensive community outreach initiatives to facilitate seamless connections between testing and treatment programs.

Collaborates closely with the HCT teams to guarantee seamless follow-up for HIV clients by conducting home visits, phone calls, and text messages, while meticulously monitoring and recording process outcomes.

Establish and lead community support groups specifically for people living with HIV (PLH IV), fostering collaboration with NEPHRON and other civil society organizations (CSOs) to enhance synergy across State, LGA, health facility, and community levels.

Implement community adherence monitoring to ensure compliance with established protocols.

Ensures the successful execution of Positive Health, Dignity, and Prevention (PHDP) principles at the community level.

Collaborates extensively with healthcare professionals and community-based or faith-based organizations to support patients throughout the entire continuum of care, ensuring sustained engagement and retention.

Facilitate seamless referrals for newly identified people living with HIV by guiding them through the transition from community-based support to facility-based care while ensuring consistent engagement and retention in health facility services.

Collaborates extensively with testing teams to maintain seamless patient progression through the care continuum and enhance client retention.

Our team is responsible for overseeing the execution of strategic HIV referral and linkage protocols within AHF health facilities to ensure seamless patient transitions and continuity of care.

Develops and enhances the capacity of healthcare workers (HCWs) and staff at community-based organization (CBO) partners, focusing on improving referral and linkage processes.

Collaborates effectively with key community entities, including the Ward Development Committee (WDC), Village Health Workers (VHW), Health Committees (HC), and Community Development Groups (CDGs).

Effectively manage data and oversee the accurate collection process at both the facility and community levels, utilizing suitable data tools.

Execute additional tasks and responsibilities as assigned, as necessary, to support operational objectives.

Qualifications

Seeking candidates with a bachelor’s degree in a relevant field or equivalent professional experience, demonstrating a solid foundation in theoretical knowledge and practical skills. Proven ability to apply educational background to real-world scenarios, with a minimum of two years of hands-on experience in a related industry preferred. Familiarity with industry standards, best practices, and emerging trends is essential for success in this role.

A Bachelor of Science degree, Bachelor of Arts degree, or any equivalent qualifications are required.

A minimum of two to three years of hands-on experience delivering HIV-related services within community and healthcare settings is required.

Proficiency in collaborating with CBO (Community-Based Organization) partners is advantageous.

Proficient communication in multiple languages is essential for this role, requiring fluency in at least two languages, one of which must be English. The ideal candidate will possess strong written and verbal proficiency in these languages, enabling effective interaction with diverse stakeholders. Additional language skills, such as Spanish, French, or Mandarin, are highly advantageous and may be required depending on the specific project or team needs.

Proficient in spoken and written English with exceptional fluency.

Proficient in evaluating, comprehending, and extracting key insights from scientific and technical journals, financial statements, and legal documentation.

Capable of addressing routine inquiries and complaints from customers as well as regulatory bodies in a timely and professional manner.

Proficient in crafting speeches and articles suitable for publication, ensuring adherence to specified style and format guidelines.

Proficient in a variety of technical and interpersonal competencies, the ideal candidate will demonstrate exceptional analytical thinking, adaptability in dynamic environments, and a commitment to continuous learning. Strong written and verbal communication skills are essential for effectively conveying complex ideas, collaborating across teams, and fostering productive relationships with stakeholders.
