The Linkage Coordinator plays a pivotal role in enhancing the integration and effectiveness of HIV testing, referral, linkage, retention, and community support systems to guarantee uninterrupted access to HIV prevention, care, and treatment services. This position oversees the execution, monitoring, and evaluation of referral and linkage processes between communities and healthcare facilities, ensuring that individuals newly diagnosed with HIV (PLH IV) are promptly enrolled in care and consistently maintained on treatment regimens. Collaboration is central to this role, as the incumbent partners with healthcare professionals, community-based organizations, faith-based groups, support networks, and local community structures to facilitate client follow-up, promote adherence, and deliver positive health dignity and prevention (PHDP) interventions alongside continuum of care services. Collaborates closely with HCT teams to ensure efficient follow-up for HIV clients, utilizing home visits, phone calls, and text messages to track and document outcomes effectively.
Establish and lead community support groups tailored for people living with HIV (PLH IV), fostering collaboration with NEPHRON and other civil society organizations (CSOs) to strengthen synergy across State, Local Government Area (LGA), health facility, and community levels.
Ensure the effective execution of community adherence monitoring initiatives.
Ensures the successful integration of Positive Health, Dignity, and Prevention (PHDP) principles within community settings.
Collaborates effectively with healthcare professionals and community-based organizations (CMOS)/faith-based organizations (FBS) to maintain patients’ engagement throughout the continuum of care and ensure their retention in treatment programs.
Facilitate smooth referrals for newly identified people living with HIV (PLH IV) from community settings to healthcare facilities, while ensuring clients remain engaged and retained within the health system.
Collaborates proactively with testing teams to maintain seamless patient care throughout the care continuum and sustain client retention.
Implements strategic HIV referral and linkage protocols at all AHF health facilities to ensure seamless execution.
Assists in strengthening the capabilities of healthcare workers and staff at community-based organization partners concerning referral and linkage procedures.
Collaborates extensively with key community organizations, including the Ward Development Committees (WDC), Village Health Workers (VHW), Health Committees (HC), and Community Development Groups (CDGs).
Oversee the meticulous organization and administration of data while directing the accurate collection of information at both the facility and community levels through the implementation of appropriate data management tools.
Assist with additional tasks and responsibilities as required to support ongoing operations and meet departmental objectives.
Qualifications

A bachelor’s degree in a relevant field such as B.Sc., BA, or equivalent qualifications is required.
Experienced professionals with a proven track record of 2–3 years delivering HIV-related services within both community-based and healthcare settings are sought.
Proficiency in overseeing Collaborative Business Operations (CBO) partners is advantageous.
Proficient in multiple languages, including but not limited to English, with strong verbal and written communication abilities in at least one additional language. Demonstrated capability to translate, interpret, and convey complex information accurately across linguistic boundaries. Experience in cross-cultural communication, ensuring clarity and precision in diverse professional contexts. Adaptable to varying language proficiency levels and capable of maintaining confidentiality in sensitive exchanges.
Proficient in both written and verbal English communication.
