Job Summary:
The Linkage Coordinator plays a pivotal role in enhancing HIV testing, referral, linkage, retention, and community support systems to ensure uninterrupted access to HIV prevention, care, and treatment services. This position oversees the execution, monitoring, and assessment of referral and linkage processes between communities and healthcare facilities, guaranteeing that newly diagnosed individuals living with HIV (PLH IV) are seamlessly enrolled in care and maintained on treatment. Collaboration with healthcare providers, community-based organizations, faith-based groups, support networks, and local structures is central to the role, enabling client follow-up, adherence support, positive health dignity and prevention (PHDP) strategies, and continuity of care. Additionally, the role supports training initiatives, community engagement, data administration, and reporting efforts to bolster program effectiveness and reinforce HIV service delivery across both facility and community settings.

Plays a key role in designing, executing, monitoring, and assessing HIV referral and linkage systems within both health facilities and community settings.
Ensures seamless integration and coordination of all referral processes while meticulously monitoring outcomes between health facilities and communities.
Delivers assistance to newly identified People Living with HIV (PLH IV) throughout the linkage processes, performing assessments to identify and proactively address potential barriers that may impede their successful linkage to care.
Implement integrated community outreach initiatives to facilitate the connection between testing and treatment programs.
Works collaboratively with HCT teams to ensure thorough follow-up for HIV clients by coordinating home visits, phone calls, and text messages while meticulously tracking process outcomes.
Establish and lead community support groups specifically for people living with HIV (PLH IV), cultivating an environment of mutual aid and empowerment. Collaborate extensively with NEPHRON and other civil society organizations (CSOs) to foster aligned efforts and maximize impact across State, Local Government Area (LGA), health facility, and community levels.
Drive the execution of community adherence monitoring initiatives to ensure compliance and effectiveness.
Ensures the successful execution of Positive Health, Dignity, and Prevention (PHDP) initiatives within community settings.
Collaborates extensively with healthcare professionals, community-based organizations (CMOS), and faith-based organizations (FBS) to ensure patients remain engaged and supported throughout the care continuum, from initial contact to long-term retention.
Facilitate the seamless transition of newly identified people living with HIV (PLH IV) between community-based support systems and healthcare facilities, while ensuring consistent client retention within these facilities.
Collaborates closely with testing teams to maintain patient engagement throughout the care continuum and enhance client retention.
Develops and ensures the execution of strategic HIV referral and linkage protocols across all AHF health facilities.
Facilitates the enhancement of referral and linkage capabilities among healthcare workers and staff of community-based organization partners through targeted capacity-building initiatives.
Collaborates effectively with key community organizations, including the Ward Development Committee (WDC), Village Health Workers (VHW), Health Committees (HC), and Community Development Groups (CDGs).
Effectively manage data and oversee precise collection processes at both the facility and community levels by employing suitable data tools.
Execute additional responsibilities as may be required or deemed necessary.

Proficiency in collaborating with Chief Business Officers (CMOS) or similar partners is advantageous.
Proficient in multiple languages, with a strong command of written and verbal communication in each. Demonstrates fluency in at least two languages, one of which must be English. Capable of translating complex texts accurately and efficiently. Skilled in interpreting spoken exchanges with precision and cultural sensitivity. Maintains a high level of accuracy and clarity in all language-related tasks.
Proficient in both written and verbal English communication.
