Job Summary

The Hospitalist provides comprehensive care for patients throughout their hospitalization, from initial admission to final discharge. This position emphasizes the delivery of superior, well-coordinated inpatient care, enhancing patient recovery and optimizing the utilization of hospital resources.

The Hospitalist collaborates with diverse multidisciplinary teams to oversee the treatment of both acute and chronic conditions in the inpatient healthcare environment.

This position demands a blend of clinical expertise, operational effectiveness, and seamless collaboration with cross-functional teams to elevate patient safety, satisfaction, and care continuity.

Responsibilities include managing project timelines and ensuring all deliverables are completed within specified deadlines. Deliver holistic medical treatment to hospitalized patients, ensuring thorough and attentive care throughout their stay.

Perform comprehensive evaluations of patients upon intake, formulate detailed care strategies, and oversee continuous treatment interventions to ensure optimal health outcomes.

Perform daily ward rounds and maintain precise documentation of clinical findings.

Observe and evaluate patient responses to therapy, modifying intervention strategies as needed to optimize clinical outcomes.

Oversee the treatment of acute illnesses, chronic disease exacerbations, and post-operative recovery.

Respond swiftly to urgent situations involving inpatients and instances of clinical decline to ensure timely intervention and patient safety.

Promote adherence to evidence-based clinical protocols and practices to maintain high standards of care.

Responsible for managing admission operations, ensuring accurate clerking and meticulous documentation throughout the process.

Conduct daily ward rounds and oversee the timely assessment of all patients.

Collaborate with interdisciplinary teams to develop and implement comprehensive discharge plans that facilitate seamless and secure patient transitions from care settings.

Prepare comprehensive discharge summaries, issue accurate prescriptions, and develop tailored follow-up plans for patients.

Collaborate with healthcare specialists, nursing staff, pharmacists, and allied health professionals to ensure comprehensive patient care.

Collaborate with other departments to initiate referrals and coordinate consultations as needed.

Collaborate seamlessly across emergency, inpatient, and outpatient services to maintain consistent and uninterrupted patient care throughout the healthcare continuum.

Engage actively in collaborative case reviews and interdisciplinary discussions to facilitate comprehensive analysis and decision-making processes.

Create evidence-based protocols and guidelines, then train medical officers and nurses in their implementation and application to patient care.

Follow established clinical protocols, guidelines, and evidence-based practices rigorously to ensure optimal patient care and treatment outcomes.

Engage actively in quality enhancement programs and clinical evaluation reviews to uphold and advance healthcare standards.

Oversee the tracking and evaluation of quality improvement and patient safety metrics, encompassing variables such as medication errors, readmission rates, and adverse events.

Compliance with infection prevention and control standards must be rigorously maintained.

Document clinical incidents and engage in root cause analysis (RCA) to identify underlying causes and prevent recurrence.

Ensure all medical records are maintained with precision, completeness, and punctuality.

Ensure clear and compassionate communication with patients and their families concerning diagnosis, treatment options, and prognosis, fostering understanding and trust throughout the care process.

Ensure thorough and comprehensible handovers are conducted at the end of each shift to facilitate smooth transitions and maintain operational continuity.

Facilitate adherence to accreditation standards, such as those established by the Joint Commission International.

Maintain transparent communication with patients and their families about diagnoses, treatment plans, and expected outcomes.

Display empathy, foster mutual respect, and uphold professionalism consistently across all engagements.

Facilitate seamless patient transitions from intake through to discharge, ensuring an uninterrupted continuum of care.

Contribute to the enhancement of patient satisfaction (CSAT) and experience (NPS) metrics.

Oversee the performance and coordination of medical officers to ensure optimal healthcare delivery and adherence to established protocols.

Engage in the clinical education and mentorship of junior physicians, facilitating their professional development through hands-on training and guidance in a clinical setting.

Deliver comprehensive training programs and provide ongoing mentorship to clinical staff.

Attend and actively contribute to clinical meetings, case reviews, ecosystem grand rounds, and various departmental activities.

Participate actively in ongoing professional development initiatives to maintain currency with evolving medical practices and established guidelines.

Ensure alignment with hospital policies and clinical governance frameworks.

Support bed management initiatives and enhance patient flow efficiency.

Contribute to the on-call schedules and shift rotations as assigned, ensuring coverage and responsiveness during designated periods.
