Job Summary:
The Linkage Coordinator plays a pivotal role in enhancing HIV testing, referral, linkage, retention, and community support systems to ensure uninterrupted access to HIV prevention, care, and treatment services. This position oversees the execution, monitoring, and assessment of referral and linkage processes between communities and healthcare facilities, guaranteeing that newly diagnosed individuals living with HIV (PLH IV) are seamlessly enrolled in care and remain on treatment. The role involves close collaboration with healthcare providers, community-based organizations, faith-based entities, support groups, and local community structures to facilitate client follow-up, treatment adherence, positive health dignity and prevention (PHDP) initiatives, and continuity of care services. Additionally, the incumbent contributes to workforce development, community engagement, data management, and reporting efforts to bolster program effectiveness and strengthen HIV service delivery across both facility and community settings.
The primary responsibilities and essential functions of this position include overseeing key operational tasks, managing project execution, and ensuring adherence to organizational standards. Plays a key role in designing, executing, monitoring, and assessing HIV referral and linkage procedures within health facilities and community settings.
Ensures seamless coordination of all referral processes while monitoring outcomes between health facilities and communities.
Delivers assistance to newly diagnosed people living with HIV (PLH IV) to facilitate their connection into care, conducting comprehensive assessments to identify and address potential obstacles in the linkage process.
Perform integrated community outreach initiatives to facilitate seamless connections between testing and treatment programs.
Collaborates closely with HCT teams to ensure seamless follow-up for HIV clients by coordinating home visits, phone calls, and text messages, while accurately monitoring and documenting process outcomes.
Establish and lead community support groups specifically for people living with HIV (PLH IV), while fostering strong collaboration with NEPHRON and other civil society organizations (CSOs) to enhance coordinated efforts at the State, Local Government Area (LGA), health facility, and community levels.
Ensure the execution of community adherence monitoring initiatives.
Delivers strategic oversight to ensure the successful integration of Positive Health Dignity and Prevention (PHDP) principles within community-based initiatives.
Collaborates extensively with healthcare professionals and community-based or faith-based organizations to maintain patient engagement throughout the continuum of care and ensure retention.
Facilitate seamless referrals for newly identified people living with HIV (PLH IV) between community-based and facility-level systems while ensuring sustained client engagement within healthcare facilities.
Collaborates closely with testing teams to ensure patients remain seamlessly integrated throughout the continuum of care and maintain client retention.
Implements strategic HIV referral and linkage protocols at AHF health facilities to ensure adherence and operational effectiveness.
Facilitates the enhancement of competencies among healthcare workers (HCWs) and staff of community-based organization (CBO) partners, specifically in referral and linkage procedures.
Collaborates extensively with key community entities, including the Ward Development Committee (WDC), Village Health Worker (VHW), Health Committee (HC), and Community Development Groups (CDGs).
Ensure robust data management and oversee meticulous data collection processes at both facility and community levels with appropriate data tools.
Assist with additional tasks and responsibilities as required to support operational needs.
Bachelor’s degree in accounting, finance, business administration, or a related field required; professional certifications such as CPA or CMA preferred. Minimum of five years of progressive accounting experience, with at least two years in a supervisory or managerial role. Proficiency in GAAP standards and experience with ERP systems. Strong analytical, organizational, and communication skills are essential. Ability to collaborate effectively across departments while managing multiple priorities under tight deadlines.

A Bachelor of Science (B.Sc.), Bachelor of Arts (BA), or equivalent qualifications in a related field are required.
A minimum of two to three years of hands-on experience delivering HIV-related services within community-based organizations and healthcare facilities is required.
Experience in collaborating with Community-Based Organizations (CMOS) presents a valuable asset.
Proficient in multiple languages, with a strong command of both written and verbal communication in each, is essential for this role. Fluency in at least two languages, including English, is required, with additional languages considered advantageous. The ideal candidate must demonstrate the ability to translate complex concepts accurately and adapt communication styles to diverse audiences. Excellent listening skills and cultural awareness are also critical to effectively engage with stakeholders from varied backgrounds.
