Job Summary

We seek a dedicated Hospital Social Worker to integrate our team and deliver compassionate assistance and direction to patients requiring extra support throughout their healthcare journey.

Hospital Social Workers are tasked with providing patients with guidance on available resources and conducting detailed interviews to assess their specific needs.

You will collaborate closely with patients to facilitate access to essential resources and support systems, aiding in their recovery from injuries or illnesses.

Manages daily operations to ensure smooth workflow and adherence to established protocols. Oversees project execution, monitoring progress and addressing any obstacles promptly to maintain timelines. Collaborates with cross-functional teams to align objectives and foster a cohesive working environment. Develops and implements strategies to enhance efficiency and productivity across departments. Conducts performance evaluations to assess team and individual contributions, providing constructive feedback and coaching as needed. Ensures compliance with industry regulations and company policies while identifying opportunities for process improvement. Maintains accurate documentation and reporting to track key metrics and project milestones. Serves as a liaison between senior management and operational staff to facilitate clear communication and alignment of goals.

Evaluates each patient’s distinct situation by analyzing information gathered from medical assessments and comprehensive patient interviews.

Formulate detailed strategies to facilitate patients’ adaptation to medical diagnoses and support their seamless reintegration into daily life following discharge from healthcare services.

Deliver customized assistance to each patient, carefully considering their health condition, individual strengths and weaknesses, emotional well-being, and other relevant factors.

Conduct an assessment of local support groups and related resources to identify those most suitable for each patient’s specific needs.

Assess the quality and impact of services and resources delivered to patients to measure their effectiveness and identify areas for improvement.

Maintain ongoing communication with patients throughout the diagnostic process, treatment journey, and recovery phase to accurately assess their mental and emotional well-being.

To ensure optimal patient care, stay abreast of local resources and the latest medical research findings, enabling the provision of informed and effective recommendations.

Seeking candidates with a bachelor’s degree in a relevant field, coupled with a minimum of three years of professional experience. Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint, is essential. Strong analytical and problem-solving abilities are required, as is the capacity to work both independently and collaboratively within a team setting. Excellent communication skills, both written and verbal, are a must, along with meticulous attention to detail. Prior experience in project management or a related role is preferred, as is familiarity with industry-specific software or tools. The ideal candidate will demonstrate adaptability, creativity, and a commitment to continuous learning and professional growth.

A Bachelor’s or Master’s degree in Social Work, coupled with specialized training and certifications pertinent to hospital-based social work practice, is required.
