Serving as a critical liaison, the caseworker will facilitate collaboration among children, families, caregivers, and the foundation’s diverse teams of professionals. This position entails conducting comprehensive needs assessments, designing tailored care plans, connecting families with essential support services, tracking progress, and guaranteeing each child receives individualized care that upholds their dignity and respect.
Oversee and execute a wide range of duties, including but not limited to project management, team coordination, and client relations, ensuring timely and high-quality deliverables. Analyze performance metrics, identify areas for improvement, and implement strategic initiatives to enhance operational efficiency. Collaborate with cross-functional teams to align goals, resolve challenges, and foster a productive work environment. Maintain compliance with industry regulations and company policies while upholding strict quality standards. Prepare and present detailed reports on progress, risks, and opportunities to senior leadership.
Perform comprehensive intake evaluations for children and their families to identify their unique requirements and assess qualification for available services.
Collaborate with families and specialists to develop tailored care and support plans for each individual.
Regularly assess and record the developmental milestones and progress of each child systematically.
Coordinate a range of essential services, including therapeutic interventions, medical treatment, and academic assistance, to ensure comprehensive care and support for individuals.
Champion the rights and inclusion of children with special needs within communities and institutions.
Offer compassionate emotional support and professional guidance to families and caregivers.
Responsible for ensuring the integrity and confidentiality of all case files and records, maintaining meticulous organization and accuracy throughout their lifecycle.
Engage in comprehensive case evaluations, collaborate in team discussions, and attend professional development workshops to enhance skills and knowledge.
Provide rapid response to crisis situations and implement timely interventions as required.
Must demonstrate availability for a variable schedule, encompassing weekends and evenings as needed.
Minimum of 5 years of professional experience in a related field is required. A bachelor’s degree in a relevant discipline is necessary. Proficiency in industry-specific software and tools is essential. Strong analytical and problem-solving skills are crucial. Excellent communication and teamwork abilities are required. Knowledge of project management principles is advantageous. Familiarity with regulatory compliance and quality standards is preferred.
Holds a Bachelor’s or Master’s degree in Social Work from an accredited institution.
Highly desirable is prior experience working with children who have special needs.
Proven ability to communicate effectively and build strong interpersonal relationships.
Proven capability to collaborate effectively with a wide range of family structures and cross-functional teams, including healthcare professionals, educators, and social services.
Proficiency in local child protection frameworks and disability support systems is required.
Organized professionals with strong time management capabilities are sought after for this role.
