Job Summary:
The Linkage Coordinator plays a pivotal role in enhancing HIV testing, referral, linkage, retention, and community support systems to guarantee uninterrupted access to HIV prevention, care, and treatment services. This position oversees the execution, monitoring, and assessment of referral and linkage protocols between communities and healthcare facilities, ensuring newly diagnosed individuals living with HIV (PLH IV) are promptly enrolled in care and consistently maintained on treatment. Collaboration with healthcare providers, community-based organizations, faith-based groups, support networks, and local structures is essential to facilitate client follow-up, adherence support, positive health dignity and prevention (PHDP) initiatives, and continuous care services. Additionally, the role involves strengthening program outcomes and improving HIV service delivery at both facility and community levels through capacity building, community engagement, data management, and accurate reporting.

Plays a key role in designing, executing, monitoring, and assessing HIV referral and linkage systems within healthcare facilities and community settings to ensure seamless patient transitions and continuity of care.
Ensures seamless coordination of all referral processes while monitoring outcomes between health facilities and communities.
Offers assistance to newly diagnosed individuals living with HIV in navigating linkage processes, with a focus on evaluating and addressing potential obstacles to successful linkage.
Deliver integrated community outreach initiatives to facilitate seamless connections between testing and treatment programs.
Collaborates closely with HCT teams to facilitate follow-up visits for HIV clients, employing home visits, phone calls, and text messages to ensure thorough tracking of process outcomes.
Establish and lead community support groups specifically for people living with HIV (PLH IV), fostering collaboration with NEPHRON and other civil society organizations (CSOs) to enhance synergy across State, Local Government Area (LGA), health facility, and community levels.
Ensure the successful execution of community adherence monitoring initiatives.
Ensures the seamless integration and execution of Positive Health, Dignity, and Prevention (PHDP) principles within community settings.
Collaborates extensively with healthcare providers and community-based or faith-based organizations to maintain seamless patient engagement throughout the care continuum and retention efforts.
Facilitate seamless assisted referrals for newly identified People Living with HIV (PLH IV) between community-based and healthcare facility systems, while ensuring sustained client engagement and retention within the respective health facilities.
Collaborates proactively with testing teams to maintain seamless patient support throughout the care continuum and enhance client retention.
Develops and oversees the execution of strategic HIV referral and linkage protocols at AHF healthcare facilities to ensure seamless patient transitions and continuity of care.
Facilitates the enhancement of referral and linkage capabilities among healthcare workers and community-based organization staff through targeted capacity-building initiatives.
Collaborates effectively with key community organizations, including the Ward Development Committee (WDC), Village Health Workers (VHW), Health Committees (HC), and Community Development Groups (CDGs).
Effectively manage data and oversee the accurate collection process at both the facility and community levels using suitable data tools.
Execute any additional tasks or responsibilities as assigned by management, as circumstances require.
Bachelor’s degree in Computer Science, Software Engineering, or a related technical field is required. Additionally, a minimum of three years of professional software development experience is necessary. Proficiency in multiple programming languages, including but not limited to Java, Python, or JavaScript, is essential. Strong problem-solving skills and the ability to write clean, efficient code are critical. Familiarity with software development methodologies such as Agile or Scrum is highly desirable. Excellent communication and collaboration skills are needed to work effectively within cross-functional teams. Prior experience with cloud platforms like AWS, Azure, or Google Cloud is a plus.

Seeking candidates with a minimum of a high school diploma or equivalent, supplemented by a minimum of two years of relevant work experience in a related field. While formal education can substitute for experience, a combination of both is preferred. A Bachelor of Science (B.Sc.), Bachelor of Arts (BA), or any equivalent qualifications are required.
2+ years of direct experience delivering HIV-related services within community-based organizations and healthcare facilities is required.
Experience in collaborating with Community-Based Organization (CBO) partners is advantageous.
Fluent proficiency in multiple languages is essential, with a priority on English and Spanish. Candidates should possess strong written and verbal communication skills in these languages. Additional languages are considered advantageous.
Proficient in both written and verbal English communication.
Capable of evaluating, comprehending, and deriving insights from standard scientific and technical publications, financial statements, and legal texts;
Proficiency in addressing routine inquiries and concerns from customers as well as regulatory authorities is essential.
Proficient in crafting speeches and authored articles suitable for publication, ensuring strict adherence to established style guidelines and formatting standards.
