Job Overview:
We are seeking a dedicated and experienced Care Manager to join our growing team. As a Care Manager, you will be responsible for the day-to-day operations of a care setting, including staff management, budgets, developing care plans, and quality of care. You will also work to ensure that clients receive the best possible care. This role requires strong organizational, communication, and interpersonal skills, as well as a genuine passion for providing exceptional care.
Responsibilities:
Staff Management:
Recruiting, supervising, and training staff.
Evaluate the performance of caregivers, providing guidance and support as needed.
Budgeting:
Managing budgets and contracts for care services.
Manage client budgets and resources effectively.
Quality of Care:
Ensuring that care meets national standards and is delivered at a high quality.
Coordinate and schedule care services, including personal care, medication management, and other necessary support.
Client Care:
Building relationships with clients and their families, and ensuring they receive the necessary medical services.
Conduct comprehensive assessments of clients’ medical, psychological, and social needs.
Develop, implement, and regularly update individualized care plans in collaboration with clients, their families, and healthcare professionals.
Educate clients and their families about medical procedures, treatment options, and care instructions.
Community Partnerships:
Building partnerships with other community organizations.
Liaise with healthcare professionals, including doctors, nurses, and therapists, to arrange necessary medical services.
Strategic Decision Making:
Making decisions about the future of the care service.
Financial Planning:
Helping clients navigate insurance coverage and government assistance programs.
Qualifications:
Bachelor’s degree in Nursing or a related field. Registered Nurse (RN) license or equivalent certification is highly desirable.
Certified Care Manager (CCM) credential is an advantage.
Minimum of 5 years proven experience as a Care Manager or in a similar role within the healthcare industry, preferably in a home healthcare setting.
Strong knowledge of home healthcare principles, practices, and regulations.
Excellent assessment, care planning, and case management skills.
Ability to supervise and motivate Caregivers.
Proficiency in Microsoft Office Suite and healthcare management systems.
Excellent communication and interpersonal skills.
Strong organizational and problem-solving abilities.
Compassionate, empathetic, and client-centered approach.
Ability to work independently and as part of a team.
Additional Requirements:
Willingness to travel locally to clients’ homes as needed.
Understanding of the cultural and social dynamics within Nigeria’s healthcare landscape.
